This page describes your rights as a client of Current Clinical Studio, PLLC, how your information is protected, and how to raise a concern — with this practice or with the state licensing authority. A fuller informed-consent document is reviewed with you before services begin.

Your Rights as a Client

As a client of this practice, you have the right to:

  • Ask questions about the work at any time — about the approach being used, the reasoning behind it, or anything else — and receive a direct answer.

  • Decline any topic, approach, or recommendation. Nothing in this work proceeds without your agreement.

  • End counseling at any time, for any reason, without penalty.

  • Receive a copy of your records. Records kept electronically are provided within 15 business days of a written request, as Texas law requires.

  • Be treated with dignity, without discrimination, and with respect for your values, culture, and decisions.

  • Receive clear information about practice policies, including fees, cancellations, communication expectations, and the scope of documentation the practice does and does not provide.

Confidentiality and Its Limits

What you share in counseling, and the records of that work, are confidential under Texas and federal law. Information is not released without your written, signed authorization stating what is being released, to whom, for what purpose, and for how long.

Texas law requires or permits disclosure without your authorization in a small number of specific situations:

  • Suspected abuse or neglect of a child. Texas law requires a report.

  • Suspected abuse, neglect, or exploitation of a person who is elderly or has a disability. Texas law requires a report.

  • A probability of imminent physical harm to you or to another person, in which case limited disclosure to medical or law enforcement personnel is permitted.

  • A court order requiring disclosure.

These are legal duties that apply to every licensed clinician in Texas. If one of these situations ever applies, only the information the law requires is disclosed.

Privacy of Your Health Information

Current Clinical Studio, PLLC, maintains client records in a secure electronic health record system and delivers counseling by secure, HIPAA-compliant video. In accordance with Texas Health and Safety Code §181.154, please be aware that your protected health information is subject to electronic disclosure — for example, in the ordinary course of treatment, payment, and health care operations. Texas law prohibits any other electronic disclosure of your protected health information without a separate authorization from you for each disclosure.

Any AI-assisted tools used in this practice operate only within its secure, HIPAA-compliant systems, never replace clinical judgment, and are reviewed with you at intake.

Records Access, Retention, and Custody

Access to Your Records

You have the right to request access to information in your clinical record. Requests must be submitted in writing. In most cases, you may obtain either:

  • A copy of your record, or

  • A clinical summary or treatment letter, which may be more appropriate for coordination of care or specific purposes (for example, sharing information with another provider).

Records maintained electronically are generally provided within 15 business days of receiving your written request, as Texas law requires when the system can fulfill the request.

Limits on Access

In limited situations permitted by law, access to some information may be restricted. If that occurs, the decision and the reason will be documented, you will receive a written explanation, and you have the right to have the decision reviewed.

Fees for Copies or Summaries

Records you access through the secure client portal are provided at no cost. If you request a prepared summary or treatment letter, a reasonable, cost-based fee may apply, covering preparation and delivery. You will be told the fee in writing before any records are prepared, and no fee is charged without your agreement.

No fee is charged for records requested by you, or by your attorney or authorized representative, in support of an application for — or an appeal of a denial of — disability-related benefits or assistance, including Social Security disability and supplemental income benefits, Medicaid, Medicare, and veterans’ benefits, as Texas law requires.

Record Retention and Custody

Clinical records are maintained for at least seven years after your final appointment, or longer where required by law or contractual obligation. Records are stored in a secure electronic health record system and safeguarded against loss, destruction, and unauthorized use.

If the practice closes, or if the clinician becomes unable to continue providing services due to death or incapacity, arrangements will be made for the secure custody of records and for reasonable client access during the retention period. Information about how to access records in that situation will be made available on the practice website or through a recorded phone message.

Communication and Messaging

Email and text are used for scheduling and logistical matters only (for example, rescheduling, confirming appointment times, or brief general questions). Telephone is used for consultations, scheduling, and other brief logistical matters. Current Clinical Studio, PLLC does not provide therapy or clinical evaluation by email, text, or telephone. Please do not send clinical questions, therapy content, or crisis information by email or text.

The secure client portal is used for requesting and managing appointments and for brief logistical communication. Please do not send clinical questions or detailed therapy content via the portal. Clinical concerns are addressed during scheduled therapy sessions.

Messages and voicemails are returned, usually within 1–2 business days. These channels are not monitored in real time and are not appropriate for emergencies. If you are in crisis, please call or text 988 (Suicide & Crisis Lifeline) or call 911.

Scope of Documentation and Third-Party Forms

Current Clinical Studio, PLLC maintains clinical records and, when appropriate, can provide treatment summaries or letters for coordination of care with other health care providers.

This practice does not complete certain types of third-party paperwork, including but not limited to:

  • Emotional support animal (ESA) letters

  • Family and Medical Leave Act (FMLA) forms

  • Work excuse, fitness-for-duty, or disability determination forms

These requests often involve a separate evaluative role and may create role conflicts with the ongoing therapy relationship. If you need this type of evaluation or documentation, you will be encouraged to seek a provider who offers those services or a separate evaluator qualified to complete them.

This practice serves only as a fact witness regarding services actually provided and does not perform forensic evaluations, capacity or competency assessments, guardianship or conservatorship evaluations, or custody evaluations, and does not offer expert opinion testimony. Requests of that kind are declined and the requesting party is referred to an independent forensic evaluator.

Filing a Complaint

Current Clinical Studio, PLLC, and its clinician are licensed and regulated by the Texas Behavioral Health Executive Council. If you have a concern about the professional conduct of a licensee, you may file a complaint with the Council.

Notice to Clients

The Texas Behavioral Health Executive Council investigates and prosecutes professional misconduct committed by marriage and family therapists, professional counselors, psychologists, psychological associates, social workers, and licensed specialists in school psychology. Although not every complaint against or dispute with a licensee involves professional misconduct, the Executive Council will provide you with information about how to file a complaint. Telehealth

Counseling is provided by secure video to clients located in Texas at the time of their appointment, and in person in senior living communities in the DFW area, upon mutual agreement of client and clinician. Before telehealth services begin, we review how sessions work, what happens if technology fails during a session, and how emergencies are handled — all of it is part of your intake paperwork.
